As a primary care provider, please recognize that time and resources are limited. In rural, we have limited staff and often no one to give a warm hand off to. I agree with the intent of the toolkit but it all needs to be able to be accomplished in a 15-20 minute appointment along with screening for CAD, breast and colon cancer, cervical cancer, and the multitude of other issues that it seems like primary care needs to address. I think the tool kit has value and I thank you for this.
